Chiropractic care for beginners

My Chiropractic clinic in San Diego primarily serves those who have had experience with chiropractic treatment over the course of their lives.  Many of them come for treatment not because they have a particular ache or pain but because they simply like the way they feel after a treatment.  I don’t want to imply that chiropractic care is addicting like smoking is, but after you have experienced the improved range of motion, increased athletic performance and decreased muscle tension that results from chiropractic care, many simply want that feeling to be the norm.  Other patients come specifically for treatment of neck pain, back pain, headaches, etc.

One thing that most of these patients have in common is that they usually know someone who could benefit from chiropractic care but that person is hesitant.  They are hesitant to come for treatment either because they don’t “believe” in chiropractors or because they are afraid.

Those who don’t believe in chiropractic care are easy to convince of the benefits of chiropractic treatment.  Many of these people hold on to the untruths spread by the American Medical Association (AMA) regarding chiropractors.  Between the 1960s and the mid 1980s, the AMA spent nearly $100,000,000 on an anti-chiropractic campaign.  While most of the information that the AMA dispersed regarding my profession was false and based on the poor behavior of just a few doctors, some people continue to hold on to those false ideas. 

That being said, chiropractic treatment has a direct and immediate beneficial effect on the joints of the spine.  This benefit is a combination of increased joint motion, relaxed muscles and decreased tension and is hard to overlook even by the most pessimistic skeptic. 

Those who are fearful of chiropractic care are more difficult to convince of the benefits of treatment.  They are afraid that they might get paralyzed or have some other life changing occurrence as a result of care.  In reality, chiropractic care is very safe.  The only “side effect” that we have personally seen regarding chiropractic care is that the patients feel better. 

The safety of your doctor’s office can be determined by the cost of their malpractice insurance.  Chiropractors insurance rates for the same level of coverage is less than 1/10 of most MDs.  What that means to patients is that despite the fact that some people don’t like the thought of having their joints “cracked”, a chiropractor’s office is a very safe place to receive treatment.

Is is ok to crack my own back, neck?

Is it OK to Crack it Myself?

In my San Diego chiropractic clinic, I treat many patients for neck pain, back pain and other joint pains.  A few of these patients ask me if it is OK if they crack their necks and backs by themselves.  A few of them will tell me that they have their kids or the wife walk on their back.  And a few of them are in my office because either they cracked their back or neck by themselves or they had someone else do it for them which resulted in an injury.

The funny thing about some of these people who were injured by a non-chiropractic adjustment is that they are nervous about me adjusting them for their neck pain or back pain.

The truth about cracking your own neck or back is that it is not very safe.  Usually people develop pain or stiffness in the spine because of an injury or some ongoing condition.  Chiropractors are trained to specifically diagnose and provide treatment for these conditions or injuries.

Through the course of my fifteen year career, I have seen patients who have broken vertebra by adjusting their own backs, I have seen broken ribs as a result of having someone walk on their backs, I have seen two patients who suffered mild strokes from cracking their own necks and I had one patient whose brother suffered paralysis from the chest down because his partner walked on his back resulting in a crushed vertebra and a seriously damaged spinal cord.

Some people crack their own joints daily for years without suffering obvious serious consequences.  Even these people are doing themselves a disservice.  In no normal situation should you feel as though you have to crack a specific joint every day.  Those who do crack the same joints everyday for years will develop arthritis in those joints resulting in pain, dysfunction and possibly disability.

In most cases cracking or as we chiropractors call it “adjusting” a joint is fairly easy once the skills are acquired.  However, the one of the most important things regarding the delivery of any type of health care is when not to deliver it.  In every case that I have seen when a patient was injured by a non-chiropractic adjustment, the injured patient “thought” that they knew what was wrong.

Don’t make the same mistakes.  If you need to get adjusted, go to the chiropractor!

Does chiropractic really work?

Does Chiropractic Work?

As a chiropractor, I am always asked “Does chiropractic really work?”  This is not only an unfair question; it is also a loaded question.     Chiropractic care is a very effective form of treatment for some traditional symptoms like neck pain and back pain and not for others.

Being a Chiropractor in San Diego, I see sports related injuries all year round.  Running injuries, falls from bicycles or rollerblades, golf injuries, basketball injuries – you name it and most likely we have seen it.  More often than not these injuries result in typical sprain / strains that result in neck pain, back pain, sciatica etc. 

The types of injuries that I just listed are a few of many joint injuries / symptoms that chiropractic care is very effective in treating.  You might say that chiropractic care was made for such injuries.  The reason that I convey this is because of the nature of these types of injuries. 

The typical sprain / strain injury usually results when a fall or an accident forces a joint through an excessive or abnormal range of motion.  This type of injury stretches the ligaments and muscles that move and support the spine.  The result of this type of injury is irritation and inflammation of the joint and scar tissue formation. 

Without chiropractic treatment, the scar tissue from sprain / strain injuries restricts and or alters normal joint motion which can cause pain and stiffness.  Chiropractic manipulations help to restore full, normal joint motion by breaking down scar tissue that would otherwise cause stiffness and pain.

Chiropractic care is an excellent form of therapy for reducing the effects of postural stress from sitting at a computer, poor lifting habits, poor posture and a variety of other stresses that we expose ourselves to on a daily basis.

So, does chiropractic work? 

The only way to know is go visit one and see.  Chiropractors want their treatment to work and are reluctant to take on cases that may not properly respond to their therapy.  No doctor that I know wants to have unsuccessful treatment sessions during the course of their work day. 

A good chiropractor can look at your history, perform an examination and make a sound determination on how well you will respond to care.  That being said, we are known as the “back doctors” so if you have neck pain, back pain or any spinal related pain, a visit to the chiropractor may be just what you need.

What if I told you that over the last 15 years I have helped countless low back pain sufferers overcome their pain with a fast, natural, drug free therapy?  In addition, that therapy is reasonably priced and covered by most insurance companies.

Of course I am writing about safe and effective chiropractic care. 

At this point, some of you will be thinking “Chiropractic doesn’t work!” or “I wouldn’t go to one of those guys!”

Kaisers’ treatment was not helpful!

Eventually I saw a chiropractor who had my condition completely turned around in about 3 weeks.  Mind you that the chiropractic care followed 5 months of pills and occasional physical therapy from Kaiser – that didn’t work. 

My chiropractic treatment was so successful and actually enjoyable that I switched my college studies to pre-med and later attended Chiropractic College.

What does this have to do with low back pain you ask? 

Everything. 

Chiropractors are known for treating low back pain, it is what my profession is associated with.  The reason that we are associated with low back pain treatment is because of my professions’ tremendous success rate with delivering fast, safe, natural, drug free back pain relief.  

I recently read a study that indicated that the use of chiropractic care for low back pain relief is nearly 4000 times safer than using medications or other traditional western medicine inspired treatments.

Most cases of low back pain occur as a direct result of injury or dysfunction involving the joints and muscles of the low back.  These are biomechanical problems that are not efficiently treated with medication (drugs).  Biomechanical joint problems account for 80% of low back pain cases and usually worsen with age. 

Chiropractic care and its use of specific joint adjustments is the logical and efficient treatment for biomechanical joint problems that are responsible for most cases of low back pain.

Have you been to a chiropractor lately?

Have you ever been to a chiropractor?
Regardless of your answer to these questions, do you know what a chiropractor does?

I have been a practicing chiropractor in San Diego for the last 16 years.  Helping people with neck pain, back pain, sciatica and headaches with chiropractic care is rewarding.  As  a profession, chiropractors only provide care to somewhere around 10% of our population.  I have always wondered why this number is so low since patient satisfaction with chiropractic treatment is so much higher than it is with traditional medical care and that most independent research indicates that chiropractic care is more effective than western medicine for back pain. 

Chiropractic care is an excellent , conservative treatment for many types of back injuries including those caused by auto accidents and various work injuries.  That being the case, I have always thought that more people don’t seek out chiropractic care for their back and neck injuries because they don’t know what to expect with a chiropractic visit.

In this post, I am going to tell you exactly what procedures that our new patients go through at the time of their first visit. 

Once a new patient comes to our office they are asked to fill out a brief informational health history that covers both past and current problems.  After they are done with this form, I review the history to acquaint myself with current and past complaints that are relevant to the reason for the appointment.

With the history form reviewed, I take the patient to one of my exam / treatment rooms where I ask the patient to tell me more about their pain.  During this time I also ask additional questions to help me get a good understanding of the patients’ concerns. 

After I am satisfied with my understanding of the patients’ concerns and they have nothing else to add to the history, I perform an examination on the involved areas that includes range of motion tests, necessary orthopedic tests and neurological tests.  Usually the information that I obtain from these tests is sufficient to allow me to make a decision regarding treatment.

If treatment is indicated, care usually involves therapy to relax the muscles followed by chiropractic adjustments to relieve the pain / dysfunction of the involved joints.  Since many of our new patients have never been to a chiropractor before, I always ask the patients to help me keep all of our procedures within their “comfort zone”.  I have always thought that this is very important since so many Americans are unfamiliar with manual type therapies.

The number of chiropractic visits varies person to person depending on the severity of the injury and each persons response to care.

Core Strength Back Pain

Being a practicing chiropractor in San Diego for the last 15 plus years has given me the opportunity to manage hundreds of cases of lower back pain.  I would say that of these cases of low back pain, at least 75% of them are the result of a sedentary lifestyle and lack of core strength.

Our spines are essentially a stack of bones that give use the ability to flex, bend and twist while at the same time provide protection for our spinal cord.  Ligaments, which are tough fibrous bands of tissue help provide stability of the joints.  Another major contributor of stability for our spines comes from the muscles.  The muscles in our bodies, regardless of where they are located should be strong and flexible.  Strong flexible muscles in our core help prevent injuries.

The trend in America is that we are becoming heavier and exercising less.  In addition to this trend, or perhaps a contributor to this trend is the fact that so many people have work that revolves around the computer.  I know that some of my patients sit at a computer station all day long – every day of the week. 

Sitting at a computer has multiple ill effects on the health of our spines.  The postural stress of sitting puts stress on the discs that separate our vertebrae.  When we sit, the normal forward sweeping curve of our low backs round backward which displaces the weight of our bodies forward putting increased pressure on the discs.  This posture also stretches the muscles that run down the back of the spine eventually causing what is known as a “stretch weakness”.

It is the loss of core strength combined with the postural stress of sitting at work for hours and days on end that leads to most of the back problems that I see at work.  I always find it ironic that office workers who are in for treatment make comments about how I must see so many heavy laborers for back pain while my office is filled with computer jockeys.

The solutions to the problems described above are simple but require diligence.  First of all, you have to take breaks if you sit all day.   I had a seminar years ago where one of the speakers gave a simple solution to get you up and moving during the day.  He suggested that you have a small, 8 oz glass of water on your desk that you drink from all day.  With this plan, every twenty minutes or so you will have to get up to get more water or get up to go to the bathroom.  These mini breaks in your day help relieve postural stress.

The second part of taking care of your back involves building strength in your core.
This involves exercise.  Walking, jogging, bicycle riding, crunches and various exercises on a Swiss Ball will all go a long way towards building core strength and reducing your chances of lower back injury.

Dangers of Acetaminophen

Dangers of Acetaminophen:

Many Chiropractors are hard core anti-medication.  I once had a roommate that I went to Chiropractic College with who was one of these types of practitioners.  With his patients his mantra was “anti drug, anti drug, anti drug.”  Of course, that didn’t stop him from going out on the weekends and getting plastered – on alcohol – which is a drug of course, and then medicating his hangover with Tylenol the next day. 

In my San Diego based Chiropractic Center, I am not quite as negative on the uses of medications as some of my colleagues.  I always recommend using caution with taking any type of medication.  This recommendation is inclusive of the medications that are bought off the shelf from the local drug store as well. 

Each year in the United States, acetaminophen (the active ingredient in Tylenol) triggers 100,000 calls to poison control centers, 56,000 emergency room visits, 26,000 hospitalizations and 450 deaths from liver failure.  Acetaminophen causes more cases of liver failure than all other medications combined.  That is a staggering statistic from a “safe” over the counter drug.

The problems of taking acetaminophen with alcohol are well known by most.  What is not as well known are the risks of taking acetaminophen while fasting.  For some people, fasting occurs secondary to abdominal upset, nausea, vomiting and other causes of lost appetite.  It is estimated that liver toxicity due to fasting occurs in 1 in 100,000 people taking acetaminophen.  Apparently, the toxic condition does occur in people who have taken the recommended doses or less.  We tend to believe that OTC medications are safe if taken as recommended on the package – this is not always the case.

The bottom line is that you always have to be careful with medications.  Medications change the physiology in your body, which should be concerning to everyone. 

Like I stated in the beginning of this article, I do believe that medications have their place and can be taken safely.  However, I am always miffed when I hear one of my patients tell me that their friend or spouse is afraid to be adjusted by a chiropractor yet they take medication on a regular basis.  Chiropractic adjustments, after all, are very safe and conservative as far as clinical treatments go. 

The bottom line is that we always have to weigh the risks of everything we do, especially with our health.  Knowledge of all the possible risks of taking any medication is easily found on the internet.  I believe it is better to know beforehand and have the chance to make a wise decision rather than to wish you had known after the damage is done.

chronic pain vs acute pain whats the difference?

 Chronic pain is defined as pain that persists longer than 90 days beyond the course of the natural expected healing time.  As a chiropractor in San Diego, I get calls from both new and established patients on a regular basis who complain of back pain, neck pain, etc.  One of the factors that comes into play when planning a chiropractic treatment program is the duration of the patients pain. 

In general, the longer that a patient suffers with a painful condition, the longer / more intense the treatment is needed in order to fix it.  Acute or recent episodes of pain respond much quicker than do chronic conditions.


Pain is very difficult to quantify by any practitioner.  Pain is subjective in nature and completely defined by the person experiencing the symptoms, the fact that chronic pain can exist without an obvious cause makes it much more difficult to treat. 

I see the difficulties that my chronic pain patients have with their conditions on a daily basis.  Aside from the actual pain that they experience, those who suffer with chronic pain often limit their activities for fear of exacerbating their conditions.  They also have a much harder time focussing on mental tasks as the pain is a constant source of distraction.  The more severe the pain, the more difficulties these people have with attention demanding tasks.

While we strive to avoid pain, pain is a wonderful tool of the nervous system.  Pain lets us know of actual or potential tissue damage with an unpleasant sensation that directs our attention to the affected area so that we can either immediately take action to stop the process or focus on healing the injured body parts.  Chronic pain sensations are a bit more insidious.

Chronic pain often involves a decrease in the sensitivity of the nerve fibers that send messages of pain from the damaged area.  With a decreased sensitivity (threshold), the nerve is much more likely to transmit painful sensations to the brain then a healthy nerve / tissue combination. 

This activity within the nervous system is know as a wind up phenomenon and it is very difficult to stop once it has been established.  In some other cases, nerve fibers that normally don’t carry painful sensations not only begin to transmit pain signals but it is thought that they may actually generate the signals themselves.

In my office, chronic pain is often the result of untreated sprain / strain type injuries of the joints.  In untreated or under-treated sprain / strain injuries, the joint or the muscles that move the joint are laden with scar tisuue that acts to either limit or alter motion of that joint.  With time, the awkward, unnatural biomechanics of the scar tissue laden joint results in irritation, inflammation and finally pain and spasm. 

While chiropractic care is often successful at re-establishing “clean” biomechanics of a joint, it is always better to get the care in the beginning of an injury versus waiting for that injury to become chronic, with a requiring more care with a decreasing chance of complete recovery.

Because of the possibility of an acute painful condition becoming a chronic one, I always encourage my patients to take care of themselves immediately when they first start to experience pain.  I have practiced chiropractic care for almost two decades now and I can say that chronic painful conditions of the spine are devastating. 

Not only do they make you experience pain, but in most instances they limit your physical activities creating a situation that can easily degrade from inactivity secondary to pain, to deconditioning and eventually to obesity and ill health in general leading to a dissatisying lifestyle.

The bottom line on chronic pain is this – avoid it if you can.  Don’t ignore your pain.  Painful acute conditions that are left untreated can easily spiral into a chronic condition that has the potential to alter your lifestyle in a negative way.

Is Chiropractic Care Addictive?

Is Chiropractic Care Addictive?  I am asked this question on a regular basis. 

There are many people who are under the impression that once you begin chiropractic treatment you have to keep going – almost like something bad is going to happen to you if you stop your care.

I understand how some people would come to think that chiropractic care could be addictive, after all, almost all medications that people get from their medical doctors for pain is addictive.  I suppose that if you are in pain, are concerned about the addictive nature of the medications that you have taken in the past, and are going to the chiropractic for the first time that you may think that chiropractic care for pain may be addictive as well.

I can assure you that chiropractic care is not addictive but feeling good can be as powerful as an addiction.  There are certain people that are more “in tune” with their bodies than others.  Restrictions in joint motion and flexibility typically occur slowly over time and are not normally perceptible.  Once these people start care they experience full, free range of motion of their joints – and they like it.  After formal care ends, a percentage of these people notice when their joints begin to lose their range of motion.  Since they felt better under chiropractic treatment, they like to continue their care simply to feel better.  This is about as addictive as chiropractic care can be.  It is really no different from those who exercise and / or diet and feel better and therefore continue with their plan.

In closing, you can receive chiropractic care with no fear of developing a physical or physiological dependency on the treatment.  Most chiropractors, myself included, know that chiropractic care is of great benefit to the health of your spine.  Keeping your spine well adjusted is akin to keeping your teeth brushed and flossed – but it is not addictive.

 How do you think I hurt my back?

I am often asked the question "how do you think I hurt my back?"  Believe it or not, in the absence of some recent significant injury, this question isn't so easy to answer.

Many patients simply wake up with back pain that wasn't there when they went to bed, or they throw their backs out while reaching for a cup of coffee or picking a sock up off the floor.  These scenarios are typical.

The truth is that our backs are very strong.  Some of the ligaments in our backs are capable of withstanding over 700 pounds of tension.  That is a lot of force.

Yet somehow we manage to injure ourselves with mundane events.

The answer to many of these injuries involves postural stress.  Postural stress is dangerous to the health of our spines.  It can result in lost days of work, disability and worst of all - loss of recreation and fun.

Postural stress results from poor posture, awkward bending and lifting, too much sitting, poor eyesight (makes us lean forward to see what we are doing) and many other activities of daily living.

These stresses that we endure are compensated for in other areas of our spines.  However, with time and enough exposure to the stress previously mentioned, the joints in our backs become irritated and inflamed leading to muscle spasms and pain.

I like to use the analogy of stacking the straws on the camels back with injuries like this.  In other words, these instances of stress are nothing by themselves but the effects of the stress is cumulative.  As the stresses build, the body’s' chance of a breakdown increases.  Eventually you suffer a breakdown.

Chiropractic care can help you control the effects of postural stress.  When our backs hurt, that is a clear indication that inflammation is present.  Inflammation always stimulates scar tissue formation which limits your spines' flexibility and causes pain.  Chiropractic adjustments breakdown the inflammation caused scar tissue and improve your spines flexibility.  Improved flexibility keeps your spine healthier and much less likely to be aggravated by your activities of daily living.

If your job requires sitting for more than four hours per day, monthly chiropractic adjustments are very important in controlling the accumulating stress in your back.

There are other obvious steps that you can take toward improving the health of your spine and preventing episodes of back pain.  Eating a healthy diet and exercise are very important in reducing the chances of suffering a back injury.
Being careful when lifting and carrying heavy objects and making sure that you don't sit in one position for longer than twenty minutes is also helpful.

Does acute or chronic back pain have you down?

Does acute or chronic back pain have you down?  Looking for alternatives to what your current back pain management plan is?   Read on! 

Practicing chiropractic in San Diego has exposed me to a patient base that is more interested in their health then in other areas of the country.  In addition to traditional chiropractic treatment for neck pain and back pain our office offers our patients a host of recommendations for self care of their pain that can become part of a healthy lifestyle. 

In order to help control episodes of back or neck pain, immediately drink several glasses of water.  In some cases, dehydration will cause muscular aches and back pain.  The rule of thumb of drinking eight, eight ounce glasses of water per day is healthy in general.  Incidentally, headaches can be caused by dehydration as well.  If you have headaches on a regular basis, increasing your water consumption may give you relief.

Ice applications to the area of pain can also be beneficial.  Ice helps reduce inflammation which not only causes pain but can trigger local muscle spasms that lead to functional losses.  There used to be an old rule concerning icing that suggested ice for the first 48 hours and then apply heat after that.  Modern clinical studies indicate that as long as you have pain, you have inflammation.  Inflammation always responds better to ice than it does to heat.  We recommend that our patients ice their back pain or neck pain 20 minutes at a time at least two times per day with at least a 20 minute break in between sessions.  Ice with a soft gel pack through a thin piece of clothing.  Lastly, watch for frostbite – if the skin over the area that you are icing turns red or brown and scabs over you did too much.

Try to not sleep on your stomach.  Stomach sleeping puts your back into extension which compresses the joints that run down the back of your spine.  You should either sleep on your back with your knees supported with a pillow or sleep on your side in the fetal position.  Either of these two positions may be comfortable for you and allow for a better nights sleep.

Do not try to exercise out of the pain.  Exercise is vital to strengthening your back and providing stability but it needs to begin when your original pain has subsided and or is no longer aggravated by physical activity.  The joints in our spines are supported by the ligaments and muscles.  Once injured, ligaments heal slowly and may never provide the support that they did before the injury.  We can improve our muscle tone, however, through exercise to regain pre-injury stability.

One last bit of advice that I will mention in this article is nutritional supplementation.  Nutritional supplements can not only help you heal faster but can help control pain and inflammation as well.  Among the common supplements that we recommend are essential fatty acids, Glucosamine, Chondroitin, SAMe and vitamin C.

Sitting to avoid backpain

 From a postural standpoint, one of the worst things that we can do to our bodies is slouch while we sit.  Sitting seems like such an innocent, non-injurious activity.  This is far from the case.

I have been practicing chiropractic in San Diego for the last 15 years and what I have seen in practice regarding back injuries would be surprising to most people in non-medical careers.  The average person that I speak with regarding back pain and injuries is of the mind set that low back pain is the result of jobs that require heavy lifting or contact sports, golfing or car accidents.

In reality, it is the constant stress and strain of postural stress that is the biggest burden on our spines.  The postural stress of sitting for hours on end in a task chair in front of a computer can become overwhelming.  When our bodies are exposed to a specific stress over a long period of time, the effects that the stress places on our bodies has a cumulative effect.  Over the course of months or years that cumulative stress expresses itself as pain, stiffness or more serious symptoms.

When I am explaining the ongoing cumulative effects of postural stress to my chiropractic patients I like to use the classic example of the Chinese water torture.  Those first couple hundred drops of water on the forehead aren’t bothersome – but a few hundred drops more will make you truly miserable.  The same process happens to your low back as the minutes, hours, days, months and years pass with you sitting in a computer chair.

There is a chart floating around that I have come across in several texts that relates your bodies posture with the amount of stress that each particular posture generates on the discs in the lower back.  As the chart demonstrates, the level of stress increases as the subject transitions from a prone position to a standing position to seated position to a slumped forward seated position.  The chart shows the pressure in the discs to equal 100% of your body weight while standing, approximately 125% of your bodies weight while sitting and over 140% of your body weight when sitting in a forward slumped position.

The reason that sitting is so hard on your back from a postural standpoint is due to the anatomy of the lower back vertebra.  When we are standing, your lower back naturally has a forward sweeping curve with the concavity of that curve facing rearward.  That curve acts to efficiently divide the weight of your body between the discs that separate your vertebra and the joints that run down the back of your spine. 

When you assume a seated position, that curve straightens out transferring some of your weight from the posterior joints to the discs.  Slumping forward transfers even more weight resulting in more compression of the discs and more stress on the low backs discs.

As time passes, this increase pressure causes irritation of the overloaded spinal structures eventually leading to inflammation, spasm and pain.  This is the basic process with many injuries to the body that result from ongoing exposure to some irritating or stressful activity.    

How is this postural stress negated?  Well, unfortunately, all if it can’t be.  But it can be limited by practicing good posture and the right ergonomic principals.  A brief ergonomic evaluation of your work area can give you loads of valuable information that will help minimize the effects of your work environment on your body.

Sitting is a sedentary task but like most things in life, if you don’t do it properly there will be undesirable consequences.

Do you seem to get headaches whenever you get neck pain?

If so, you are dealing with a very common problem.  My San Diego chiropractic office sees people on a regular basis who suffer from these symptoms.   Neck pain is often accompanied by headaches because of the muscle tension that is so often present with neck problems.

I am sure that many of you have heard of muscle tension headaches.  More often than not, muscle tension headaches begin in the neck where tightness has developed for a variety of reasons.  Sometimes neck injuries such as those from auto accidents or postural stress secondary to sitting in front of a computer all day will cause the neck muscles to tighten up.

Tight muscles in the neck cause muscle tension on the back of the head.  This tension causes the muscles that cover your skull to spasm.  Between the aggravation of tight muscles and neck pain, headaches will often become a companion to your neck pain.

Treating muscle tension headaches often requires a multifaceted approach.  Chiropractic care will help relieve a great deal of the muscle tension and spasm.  Chiropractic adjustments relax muscles, restore normal joint motion and eliminate the associated neck pain.  In addition to chiropractic manipulations, therapies including electric muscle stimulation, ultrasound and ice packs are beneficial to help reduce the muscle spasms that help cause the tension headaches.

Another important part of elimination or reducing the number of muscle tension headaches that you experience is to identify what sets them off.  Postural stresses associated with computer work, reading, certain types of work or specific hobbies can lead to stress in the neck that causes headaches.  Identifying the culprits and either eliminating them or modifying them helps greatly in controlling your headaches.

The bottom line is that neck pain and muscle tension headaches are closely related and often successfully treated with chiropractic care.

Hip Pain means different things to different people.  I treat many people for hip pain in my San Diego Chiropractic center.

Earlier I wrote that hip pain means different things to different people.  What I meant by that is that usually the patients who complain of hip pain would point to their lower backs when I ask them to point to the pain.  When I ask them at that point if it is actually lower back pain that they are feeling they will usually think for a moment and then say, “Yes, I guess it is low back pain.”

When patients complain of hip pain they are usually referring to pain in the area of the sacroiliac joint.  The sacroiliac joints are two large joints that are positioned just to the sides of the bottom vertebra in the back right at the level of the beltline.  I believe that people refer to this as hip pain because it is typically at its worst when walking.

True hip pain found in the joint where the femur joins with the socket found in the pelvic bones.  The typical patient will identify hip pain by pointing to the front of the body, below the beltline and off to either side of the midline of the body.

Sacroiliac joint pain is often brought on or aggravated by prolonged sitting, bad lifting habits and direct injuries such as those that occur with sports or auto accidents.  The sacroiliac joints are the largest joints in the spine and can heal slowly after an injury.

Chiropractic care supported by ice pack applications, light stretching and avoidance of know aggravating activities (if it hurts don’t do it) will usually eliminate the pain.

Chiropractic care is also effective for true hip pain but the approach is somewhat different.  Since hips are joints, chiropractic care is also quite effective in relieving hip pain as well

My San Diego Chiropractic Office sees many people with lower back pain.  Sometimes that lower back pain is accompanied by sciatica or leg pain, but more often than not the pain is concentrated in the lower back

The reasons that people get lower back pain are numerous.  Sometimes there is a traumatic history such as car accidents, work injuries and sports injuries; sometimes the reasons are less obvious.  When an accident of some type is in the immediate history, there is an easy connection between that accident and the pain.  However, the other contributing factors that cause the onset of low back pain can be less obvious.

Poor posture, improper lifting techniques and lack of proper support from your chairs or car seats can silently contribute to low back pain.  Often times, when patients complain of lower back pain with no accident history, the cause of that pain is related to cumulative stress. 

Let me explain why this happens.  The joints in our backs and the muscles and ligaments that work to provide movement and stability of these joints are among the strongest in the body.  When we lift improperly or assume poor postures, we stress the joints in our backs.  Each individual stress doesn’t affect us enough to generate pain.  However, if we continually stress the same joints they will eventually become irritated and inflamed resulting in pain and muscle spasms. 

Sometimes the pain is located more on one side of the back then the other.  This pain can indicate that one of the sacroiliac joints has been affected by the injury.  Occasionally the pain starts on one side and moves to the other side causing a band-like pain across the belt line area of the low back.  This type of pain distribution is caused by the interdependence that the sacroiliac joints have on each other and the lower lumbar vertebra.  In other words, all of these joints in the low back share muscles and ligaments that have a direct affect on each other.

 How to avoid getting low back pain

Low back pain has many contributing factors.  My San Diego based Chiropractic office sees people with lower back pain on a regular basis.  Most of these people have injured their backs in the typical ways.  Improper lifting techniques, postural stress, sports injuries, work injuries and auto accidents often result in either acute or chronic back pain.  Some of these contributing factors to lower back pain are avoidable and some are not. However, there are other, more insidious contributing factors to low back pain.

Adolescents who smoke can more than double their risk of developing long-term low back pain, according to a recent study in the American Journal of Epidemiology. 

Research had previously linked chronic back pain in adults with smoking during the teenage years.  It follows then that by preventing adolescents from smoking, they may be able to avoid low back pain later in life.

Researchers have also investigated the influence of various risk factors on development of pain: explosive growth spurts; poor flexibility; weak abdominal muscles and lack of physical activity.

Studies show that 17 percent of adolescents complain of low back pain. Those who experienced a major growth spurt of more than two inches in six months were the most likely to develop lower back pain, in fact, it tripled the odds of developing pain.  Growth spurts are unavoidable and not under our control, but the other major contributors to developing pain were all preventable: smoking, poor weight lifting form, and poor flexibility are all avoidable.

Education is a good first step to keeping adolescents from developing low back pain. Make your kids aware of the risk factors for developing lower back pain, and the consequences they will face later in life. In addition, be sure they follow a few simple rules:

•    Avoid smoking.
•    Stretch adequately, especially prior to working out.
•    Strengthen the abdominals
Finally, lead by example, kids who grow up in environments that are health conscious tend to be healthier people.

In a May 12, 2003 release from the "Canada NewsWire" was some information and advice about gardening. This publication included poll results that revealed that gardening and yard work are the number one causes of back and/or neck pain in the spring and summer months. The poll was conducted by national research firm Pollara, where 500 Ontario Canada chiropractors were asked what the most frequent causes were of back and neck pain among their patients.

The results of the poll showed that Ontario chiropractors report that working in the yard and garden are the most common sources of back and neck pain they see during the warm weather season accounting for 88% of neck and back complaints. Golf ranked in second place at 31 per cent, tied with outdoor sports in general at 30 percent.

Dr. Dennis Mizel, president of the Ontario Chiropractic Association noted, "In Canada, injuries resulting from gardening cost an estimated $3.5 billion dollars. Apparently all that digging, lifting, raking, pruning, planting, weeding and watering can cause significant strain to the muscles of the neck and back." Dr. Mizel continued, "The good news is that it's preventable. Gardening can be a serious workout. That's why we're encouraging people to treat it like any other kind of exercise. Warming-up before digging in, and using the proper techniques and tools can go a long way to letting people enjoy the results of their labor pain-free."

The Ontario Chiropractic Association offered several tips for back smart gardening:

**Stretch before You Start: Warming-up your muscles with stretches before going out helps to reduce the stress and strain on your joints and muscles, reducing the chance of injury.

**Bend Your Knees to Lift with Ease: When lifting, keep your back straight and bend your knees. Always carry the load close to your body and avoid twisting.

**The right tools, the right moves: Use the right tools and moves for the job. Kneel to plant and change positions frequently when raking, digging, hoeing or pruning. Use ergonomically designed, long handled, lightweight tools.

**Take a Break before It Aches: Give yourself and your back a break. As a rule-of-thumb take a brief rest or stretch break at least three times each hour, and drink fluids frequently.

If it is, you are not alone.  National statistics point to the computer as a major contributor to lost days at work due to injury.  I see the same thing in my San Diego Chiropractic office.  Neck pain, shoulder pain, wrist pain, hand pain and back pain are common among those who don’t have the proper ergonomic set-up of their computer stations.  That being said, some people get these symptoms regardless of their ergonomic set-up.

Computer ergonomics is the study and / or implementation of workstation design with the purpose of reducing or eliminating physical stress.

My interest in repetitive stress syndromes began approximately 10 years ago when my practice started seeing a much greater number of people suffering from the effects of computer / keyboard work.  Despite all of my training and continuing education related to these types of injuries, I quickly realized that traditional treatment for this type of injury was marginally successful at best.

Any type of therapy was going to have to include the application of basic ergonomic principals to the patient’s work environment.

I am of the opinion that treatment alone or application of ergonomic principals by themselves was not going to resolve a repetitive stress injury.

I see many older (and sometimes younger) patients in my San Diego chiropractic office who have been diagnosed with osteoporosis.  Osteoporosis is an insideous  disease.

Initially it developes without any clear symptoms, affecting the skeletons’ bone density.  Osteoporosis is usually discovered by special testing or bone weakening that leads to fractures which can result in falls, back pain, neck pain, etc.

I am always surprised that those diagnosed with osteoporosis know so little about it.  I did a search on line and found the following article that is worth reading.

Osteoporosis is a disease of bone leading to an increased risk of fracture. In osteoporosis the bone mineral density (BMD) is reduced, bone microarchitecture is disrupted, and the amount and variety of non-collagenous proteins in bone is altered. Osteoporosis is defined by the World Health Organization (WHO) in women as a bone mineral density 2.5 standard deviations below peak bone mass (20-year-old sex-matched healthy person average) as measured by DXA; the term "established osteoporosis" includes the presence of a fragility fracture.[1] Osteoporosis is most common in women after the menopause, when it is called postmenopausal osteoporosis, but may develop in men and premenopausal women in the presence of particular hormonal disorders and other chronic diseases or as a result of smoking and medications, specifically glucocorticoids, when the disease is called steroid- or glucocorticoid-induced osteoporosis (SIOP or GIOP).

Osteoporosis can be prevented with lifestyle advice and medication, and preventing falls in people with known or suspected osteoporosis is an established way to prevent fractures. Osteoporosis can be treated with bisphosphonates and various other medical treatments.

Osteoporosis itself has no specific symptoms; its main consequence is the increased risk of fracture. Osteoporotic fractures are those that occur in situations where healthy people would not normally break a bone; they are therefore regarded as fragility fractures. Typical fragility fractures occur in the vertebral column, hip and wrist.
The symptoms of a vertebral collapse ("compression fracture") are acute back pain, often with radiculopathic pain (shooting pain due to compression of a nerve) and rarely with spinal cord compression or cauda equina syndrome. Multiple vertebral fractures lead to a stooped posture, loss of height, and chronic pain with resultant reduction in mobility.

Fractures of the long bones acutely impair mobility and may require surgery. Hip fracture, in particular, usually requires prompt surgery, as there are serious risks associated with a hip fracture, such as deep vein thrombosis and a pulmonary embolism, and increased mortality.

The increased risk of falling associated with aging leads to fractures of the wrist, spine and hip. The risk of falling, in turn, is increased by impaired eyesight due to any cause (e.g. glaucoma, macular degeneration), balance disorder, movement disorders (e.g. Parkinson's disease), dementia, and sarcopenia (age-related loss of skeletal muscle). Collapse (transient loss of postural tone, with or without loss of consciousness, leads to a significant risk of falls; causes of syncope are manifold but may include cardiac arrhythmias, vasovagal syncope, orthostatic hypotension and seizures. Removal of obstacles and loose carpets in the living environment may substantially reduce falls. Those with previous falls, as well as those with a gait or balance disorder, are most at risk.

Osteoporosis requires special considerations in our office.  Chiropractic techniques involving little or no force have to be utilized to ensure patient safety.  In addition to variations in technique, nutritional suppliments are also encouraged, especially mineral supplimentation.  Osteoporosis treatment requires a multifaceted approach in order to see improvements.  Patients also must show patience with treatment as results are usually only seen with prolonged periods of care.
